Patient Care Navigator

Job Purpose:
We are looking for a compassionate, patient-oriented, and highly organzied Patient Care Navigator to support patients in understanding and accessing government financial assistance programs for their medications and treatment need.
Key responsibilities:
- Assist patient throught phone calls and other channels.
- Guide patients on application process, documentary requirements, submission procedures and where to obtain required documents.
- Explain approval timelines, application limitations, frequency of availments, and other program complexities in clear and emphatetic manner.
- Educate patient understand the importance of submitting complete and accurate documentatry requirements. which may vary depending ongovernment agency.
- Coordinate and follow up with patientsregarding pending requirements and application status.
- Maintain accurate documentation and records of patient interactions and assistance provided.
- Provide a positive and supportive patient experience while handling sensitive concerns professionally.
- Preferably residing in paranaque.
- With work experience working in any of government agencies especially those providing assistanc eto patients.
- Proficient in basic computer applications and documentation.
Qualification:
College graduate (4 year course; medical or allied health graduate)
