What You’ll Do:

  • Provide personalised patient case management and ensure patients submit the required program documentation in a timely manner.
  • Conduct scheduled outbound calls to enrolled patients, manage inbound calls, and respond to FAQ-related enquiries.
  • Accurately capture and maintain all relevant patient information in the Patient Care Management System during and after interaction.
  • Ensure programs are delivered in accordance with established service levels and quality standards, including case handling time, predefined protocols, patient data confidentiality, and Standard Operating Procedures (SOPs).
  • Adhere to medical best practices and guidance provided by the Operations Manager and Medical Advisory Team.
  • Identify and report any Adverse Events (AEs) disclosed or observed during patient interections in accordance with reporting requirements.
  • Prepare periodic program reports and support audit activities, including internal and external audits, project reviews, and compliance checks.
  • Perform any other duties assigned by the Operations Manager or Program Manager

 

Must-Have: 

  • NITEC or Diploma in a related fields.
  • Healthcare or customer service internship/industrial attachment experience or relevant work experience is preferred.
  • Excellent command of spoken and written English.
  • Proficiency in a second language is an advantage.
  • Strong coaching skills, communication, and active listening skills, with the ability to motivate and support patients in achieving their health goals and adhering to program requirements.
  • Strong problem solving skills with ability to assess situations and make timely decisions.
  • A collaborative team player who thrives in a patient-centric environment.
